dc.description.abstractАвторът си поставя за цел да проучи диагностичните и прогностични възможности на биопсията на тестисите при мъже с азооспермия и успеваемостта от in vitro процедурите със сперматозоиди от различни локализации на репродуктивния тракт (при обтурационните форми) и прогностичните възможности за консервативно лечение при висок миотичен индекс в случаите с необтурационна азооспемия. Авторът си поставя за проучване 9 конкретни задачи. Изследвани са 1485 мъже. Използвани са следните методи: анамнеза, общо клинично и специализирано изследване, лабораторни, хормонални и семинологични изследвания. Проведоха се цитогенетични изследвания и PCR за диагностициране на микроделеции на Y хромозомата. При 156 мъже с азооспермия се извърши инцизионна биопсия на семенника и се направиха хистологични, хистохимични, морфометрични и количествени изследвания на биопсичния материал. Резултати. Морфологичните находки бяха класифицирани в 8 групи. Винаги се извършваше и изследване на еластичните влакна в базалната мембрана и оценка на Лайдиговия апарат. Резултатите от ICSI процедурите извършени от сперматозоиди, получени от различни локализации на репродуктивния тракт и отчетени по отношение на % на оплождане, % на регистрирани бременности и % на ражданията показаха сигнификантни различия. При извършването на корелационен анализ за проследяване на връзката между мястото, от което идва сперматозоида и трите изследвани показателя се доказа умерена връзка (p<0,01), която показва, че най-добри резултати се постигат от еякулат, на второ място са сперматозоидите от надсеменника и най-лоши са разултатите със сперматозоиди от тестиса поради тяхната незрялост и незавършен процес на спермиация. Заключение. Биопсията на тестиса си остава златният стандарт при диагнозата и прогнозата на мъжете с тежки фертилни нарушения.BG
dc.description.abstractThe objective of the present dissertation was to study the diagnostic and prognostic possibilities of testicular biopsy in men with azoospermia and the success rate of in-vitro procedures using spermatozoa from different locations of the reproductive tract (with obturation forms) as well as the prognostic opportunities following the conservative treatment in the cases with non-obturation azoospermia presenting with high mitotic index. There was a set of nine concrete tasks of the study. The investigation covered a total of 1485 male patients. The following methods were made use of: patient’s history, general clinical and specialized examinations, as well as laboratory, hormonal and seminal analyses. Cytogenetic investigations along with polymerase-chain reaction contributed to the diagnosis of Y chromosome microdeletions. Incision biopsy of the testis was performed in 156 males that enabled comprehensive histological, histochemical, morphometric and quantitative examinations of the biopsy specimens.Results. The morphological findings were classified in eight groups. In addition, in all the cases, the elastic fibers in the basal membrane were studied and Leydig cell apparatus was evaluated. The results obtained after ICSI procedures using spermatozoa from different localizations of the reproductive tract after reading in association to the percentages of fertilization, encountered pregnancies and deliveries as well demonstrated significant differences. The correlation analysis of the relationship between the location from which the spermatozoon originated on the one hand, and the aforementioned three parameters, on the other hand, proved a moderate correlation (p<0,01). It demonstrated that the best results were achieved with spermatozoa from the ejaculate. Next came the spermatozoa from the epididymis while the results of the spermatozoa from the testis were worst because of their immaturity and incomplete process of spermiation. Conclusion. Testicular biopsy remains the golden standard in the diagnosis and prognosis of men with severe fertility disturbances.
