dc.description.abstractРакът на млечната жлеза (РМЖ) е най-често диагностицираното злокачествено заболяване и водеща причина за смърт при жената в световен мащаб. Проучени са резултатите от приложението на съвременните диагностични и лечебни методи при 93 болни жени със средна възраст от 59,05±12,60 г. с двустранен РМЖ. При 39 от тях се касае за синхронен, а при 54 - за метахронен РМЖ. Имунохистохимичните изследвания на естрогеновите и прогестероновите рецептори, HER 2 и СА 15-3 са с диагностична стойност. Проведени са модерни радикални, органо-съхраняващи и реконструктивни операции в зависимост от спецификата на туморния и общия статус при всяка отделна болна. В рамките на органопластичния подход е проведено и индивидуализирано консервативно лечение: адювантна, неоадювантна и палиативна химиотерапия, пред- и следоперативно лъчелечение, хормонална и таргетна терапия. Установяват се статистически значими корелационни зависимости при използването на лъчелечението, химиотерапията и таргетната терапия след хирургичното лечение в сравнение с отказа от това консервативно лечение. Функционалните и естетически резултати при извършването на оперативната реконструкция на гърдата след радикалната мастектомия са много добри. Общата смъртност възлиза на 33,33% (31 смъртни случая). Органосъхраняващите и реконструктивни операции играят важна роля при болните с двустранен РМЖ. Използваният от нас онкопластичен подход при синхронния и метахронен РМЖ е единствената комплексна терапевтична опция с доказана целесъобразност и клинична ефективност и трябва да се прилага по-широко в българската клинична практика.BG
dc.description.abstractBreast cancer represents the most commonly diagnosed malignant disease and the leading cause of death in females worldwide. The results from the application of modern diagnostic and therapeutic methods in 93 female patients at a mean age of 59,05±12,60 years with bilateral breast cancer are analysed. In 39 females, there is synchronous breast cancer, while in 54 females, a metachronous one is present. The oestrogen and progesterone receptors' immunohistochemical examinations, HER 2 and СА 15-3 possess a diagnostic value. Several types of modern radical, organ-sparing and reconstructive operations are performed depending on the specificity of the tumour and the general status of any single female patient. Within the organoplastic approach framework, individualized conservative management including adjuvant, neoadjuvant and palliative chemotherapy, pre- and postoperative irradiation therapy, hormonal and target therapy is carried out. Some statistically significant correlation dependences when using irradiation therapy, chemotherapy, and target therapy following the surgical management are compared to the patient's refusal to apply these conservative methods. The functional and aesthetic results of the surgical mammary gland reconstruction after the radical mastectomy are very good. The total mortality rate amounts to 33,33% (31 lethal cases). Organ-sparing and reconstructive operations play an essential role in patients with bilateral breast cancer. The oncoplastic approach used by us in synchronous and metachronous breast cancer is the only complex therapeutic option with a proven purposefulness and effectiveness and should be more widely applied in Bulgarian clinical practice.en_US
