dc.description.abstractКолоректалният карцином (КРК) е злокачествено заболяване, чиято честота в последните години непрекъснато нараства. През последните години бяха разработени редица обещаващи туморни маркери, допринасящи за ранното диагностициране, прогностична оценка и мониториране на заболяването. При пациенти с КРК мутациите на p53 са едни от най-честите нарушения, отговорни за придобитата лекарствена резистентност. При блокиране на апоптозата в туморните клетки се активират други алтернативни форми на клетъчна смърт: автофагия и некроптоза. Взаимодействието между различните видове клетъчна смърт може да бъде изключително сложно. Липсата на еднопосочни резултати в достъпната литература и недостатъчния брой клинико-морфологични проучвания посветени на ролята на автофагията и некроптозата при КРК, обуславят необходимостта от допълнителен анализ на експресията на маркерите за автофагия и некроптоза при пациенти с КРК. Цел на настоящата работа е да се изследва ролята на експресията на маркера за автофагия – Beclin-1 и за некроптоза – RIP3 във връзка с клинико-патологичните показатели и общата преживяeмост при пациенти с КРК. В настоящото проучване са включени общо 107 пациенти с диагноза аденокарцином на дебело черво. Диагнозата и стадият на тумора са потвърдени според 7-мо издание на AJCC (American Joint Committee on Cаncer) на базата на хистологичното и образно изследване. При всички изследвани пациенти е проведено радикално хирургично отстраняване на първичния тумор в дебело черво с последващо провеждане на 5-FU базирана системна химиотерапия. На всички пациенти е определено нивото на клетъчна експресия на Beclin-1, а на 81 от тях – нивото на експресия на RIP3. Резултатите са обработени статистически. Научни приноси с практическо-приложен характер: За първи път у нас е определена потенциалната прогностична стойност на високата експресия на Beclin-1 за ниска обща преживяемост при пациенти с колоректален карцином. За първи път е оценена потенциалната прогностична стойност на ниската цитоплазмена и високата ядрена експресия на RIP3, маркер за некроптоза, за ниската обща преживяемост при пациенти с КРК. Едновременно е анализирана експресията на автофагията и некроптозата във връзка с клинико-патологичните показатели и общата преживяемост при пациенти с КРК. Определен е профилът на пациентите с повишен риск за смърт от КРК на базата на клинико-морфологичните характеристики, имунохистохимичната експресия на Beclin-1 и RIP3, маркери за автофагия и некроптоза. Връзката между автофагията, некроптозата и апоптозата на този етап на научните изследвания изглежда доста сложна и противоречива. Изследванията са относително малко на брой, поради което са необходими по-голям брой проучвания с цел изясняване на точните молекулни механизми на взаимодействие между различните видове клетъчна смърт и тяхната роля в неопластичния процес. Това би предоставило информация за изграждане на по-добри терапевтични стратегии за лечение на пациентите със злокачествени заболявания и в частност на тези с КРК. Едновременно с разработването на новите терапевтични схеми е необходимо да се определят ясно клинико-морфологичните прогностични маркери, скалите им за оценяване и влиянието им върху формите на клетъчна смърт.BG
dc.description.abstractColorectal carcinoma (CRC) is a malignant tumour characterized by constantly increasing incidence. Recently, a number of promising tumour markers have been developed that contribute to the early diagnosis, prognostic evaluation and monitoring of CRC. P53 mutations are among the most common disorders responsible for the acquired drug resistance in patients with CRC. When apoptosis is blocked, other alternative forms of cell death are activated in tumour cells: autophagy and necroptosis. The interaction between different types of cell death can be extremely complicated. The lack of unambiguous results in accessible literature and the insufficient number of clinical-morphological studies about autophagy and necroptosis determine the need for further analysis of the expression of markers for autophagy and necroptosis in CRC patients. The goal of this dissertation is to investigate the expression of Beclin-1 – a marker for autophagy and RIP3- a marker for necroptosis in relation to clinic-pathological indicators and overall survival in patients with CRC. This study includes 107 patients with adenocarcinoma of the large intestine, diagnosed according to the 7th edition of AJCC (American Joint Committee on Cancer). The diagnosis is confirmed by histology and imaging studies. Radical surgical removal of the primary tumour in the colon was performed on all patients. The patients were suitable for 5-FU-based systemic chemotherapy. The level of Beclin-1 expression was determined in all patients, while the expression of RIP3 was investigated in 81 patients. The results are statistically processed. Scientific contribution of an original character: A complex clinical-morphological, molecular-genetic and immunohistochemical characterization of autophagy and necroptosis was performed in CRC patients. Immunohistochemical expression analysis of Beclin-1 and RIP3 was performed to elucidate the relationship between autophagy and necroptosis to clinical-morphological parameters through several assessment scales. Practical-applied scientific contributions: The potential prognostic value of high Beclin-1 expression for low overall survival in patients with colorectal carcinoma is determined for the first time in Bulgaria. First-time assessment of the potential prognostic value of low cytoplasmic and high nuclear expression of RIP3, a marker for necrotizing low overall survival in patients with CRC. Simultaneously, the expression of autophagy and necroptosis in relation to clinical-pathological parameters and overall survival in CRC patients was analyzed. The profile of patients at increased risk of death by CRC was determined on the basis of clinico-morphological characteristics, immunohistochemical expression of Beclin-1 and RIP3 – markers for autophagy and necroptosis. The relationship between autophagy, necroptosis and apoptosis at this stage of research seems rather complex and contradictory. Research is insufficient, so more studies are needed to clarify the exact molecular mechanisms of interaction between different types of cell death and their role in the neoplastic process. This would provide information for establishing better therapeutic strategies to treat patients with malignancies, and in particular those with CRC. Together with the development of new therapeutic regimens, it is necessary to clearly define the clinical-morphological prognostic markers, their assessment scales and their impact on the types of cell death.en_US
