dc.description.abstractАлтернативен метод за лечение на фрактурата на бедрената шийка при възрастни пациенти, с травми на опорно двигателния апарат е първичното ендопротезиране на тазобедрената става. При средносрочно проследяване на 167 пациенти, стигнахме до следните изводи: 1.Еднополюстното ендопротезиране е ефективен метод за лечение при вътреставни счупвания на бедрената шийка при изследваните пациенти, даващ възможност за възстановяване на предшестващата фрактурата двигателна активност. 2.Изборът на терапевтичен метод при фрактури тип Garden III и IV се аргументира от непрогнозируемите шансове за консолидация на фрактурата поради наличната по правило исхемия на бедрената глава 3.Рехабилитационният протокол при хемиартропластика на тазобедрената става позволяващ максимално ранно мобилизиране на пациента и натоварване на оперирания крайник е предимство за индициране на предлагания от терапевтичен подход при тази група. 4.Първичната тазобедрена хемиартропластика след фрактура на бедрената шийка при изследваните пациенти демонстрира предимства по отношение на шансовете за бързо следоперативно възстановяване дори и при налична значима коморбидност. 5.Постигането на много добри терапевтични резултати зависи от фрактурните характеристики, качеството на костта, общото състояние на пациента, интервалът между инцидента и оперативната интервенция, както и от добрата хирургична техника минимизираща оперативната травма. 6.За този период на проследяване при тазобедрената хемипротеза с моноартикуларна елиптична глава се наблюдават най-добри рентгенологични и функционални резултати.BG
dc.description.abstractAn alternative method of treating femoral neck fracture in in elderly patients is primary joint replacement of the hip joint. In this study we have set ourselves the following objectives: - To evaluate the therapeutic efficacy of unipolar femoral head replacement in unstable intra-articular neck of femur fractures in patients over 75 years of age. - To propose a therapeutic algorithm aimed at minimizing the perioperative risk and optimizing the period of functional recovery of patients in this age group. For the period from 2011 to 2015 we performed 432 primary unipolar arthroplasties. Subjects of the study were 209 patients selected according to the age criteria. For a two-year post-operative period we managed to follow up 167 of the cases – 105 women and 62 men. The multiple factor analysis of the achieved therapeutic results in our study in the mid-term follow-up of 167 patients with hip hemiarthroplasty for a femoral neck fracture gives us the basis to draw the following conclusions: The unipolar endoprosthesis is an effective method of management for intra-articular neck of femur fractures in patients over 75 years of age, enabling the recovery of the motor activity preceding the fracture. The selection of a therapeutic method in Garden stage III and Garden stage IV fractures is justified by the unpredictable chances of fracture consolidation because of the present femoral head ischaemia and is an indication of primary joint replacement of the hip in elderly patients. The rehabilitation protocol for hip hemiarthroplasty that allows for a very early mobilization of the patient and exertion of the operating limb is an advantage and an essential argument for the indication of the proposed by us therapeutic approach in this patient group. The primary hip hemiarthroplasty after neck of femur fractures in elderly patients is an advantage regarding the chances of rapid recovery even in significant comorbidity, although having a higher post-operative risk compared to conventional osteosynthetic treatment methods. Achieving very good and excellent therapeutic results depends not so much on the design of the endoprosthesis but on the fracture nature, the quality of the bone, the general condition of the patient, the interval between the incident and the operative intervention, and last but not least the good surgical technique that minimizes the operative trauma. In the mid-term follow-up period the best x-ray and functional results are observed in hip hemiarthroplasty with a mono-articular elliptical head, probably due to its "physiology", which adduces arguments for its recommendation as an implant of choice in this therapeutic method.en_US
