dc.description.abstractЦел: Да се съпоставят ехокардиографски показатели при пациенти с Таласемия майор(ТМ) и здрави контроли; да се корелират тези показатели с биомаркер за сърдечна недостатъчност- NT-proBNP, за да се обхванат рано тези пациенти, при които има данни за сърдечна недостатъчност и да се оптимизира лечението. Материали и методи: проследихме 37 пациента с хомозиготна бета-ТМ и 50 здрави лица, ехокардиографско изследване- 2Д, М-мод, пулсов и тъканен Доплер, стрейн, стрейн рейт и NT-proBNP.Резултати: Стойността (ЛММи), относителната дебелина на стените са увеличени при пациентите с ТМ; миокардната тъканна скорост (Sm), стрейн и стрейн рейт са сигнификантно по-ниски от тези на контролната група; индексираният обем на лявото предсърдие (мл/м?) е увеличен, а левопредсърдната фракцията на изтласкване е намалена; стойността на NT-proBNP (pg/ml) са значително по-високи при ТМ 80,4 (38,57-189,25), в сравнение с контролната - 23 (20- 32,80), (р<0,0001). В групата на пациенти с ТМ, 11 (29,7%) са с над граничната, 125 pg/ml, стойност за диагноза СН. При сравнение на ехокардиографските показатели между двете подгрупи в пациентската кохорта- с нормална и с повишени стойности на NT-pro-BNP, единствената разлика е индексираният обем на лявото предсърдие(ЛПОи), р-0,025. Установява се корелация между ЛПОи и NT-proBNP- r=0.44, p=0.009, и между ЛММи и ЛПОи, r = 0,480, p= 0,004. Извод: Единствената значима разлика между групата с неповишена и с повишена стойност на NT-proBNP е ЛПОи, показател, който може да се използва при рутинното ехокардиографско проследяване на пациентите с ТМ.BG
dc.description.abstractCardiac involvement still remains a leading cause of morbidity and mortality among patients with transfusion-dependent Thalassemia Major (TM) despite recent advances in therapeutic management. Myocardial iron overload together with increased cardiac output are considered the principal causes of cardiac complications especially for the development of heart failure, although patients are regularly transfused and at optimal chelation therapy. The aim of the present study is to detect early signs of cardiac dysfunction before overt heart failure takes place. Overall, 87 participants were included – 37 in the TM group and 50 controls. TM patients were slightly older – 32.35±10.93 vs 31.06±8.46 years, р=0,55, and half of them are females – 54% vs 50%, р=0.71. It is very well known from a clinical standpoint how difficult it is to distinguish symptoms of easy fatigability and effort dyspnoea in patients with chronic anaemia, heart failure or both as is the case in TM patients. The patients with TM have hemodynamic, structural, and functional changes both in systole and in diastole. This was most evident in left atrial volume index increase and NT-pro-BNP elevation. Since all indices were significantly different from healthy subjects but within normal range or borderline regular echocardiographic follow-up of TM patients with NT-pro-BNP determination is clinically beneficial for early detection of deterioration of cardiac function. Application of strain is an important method for detecting LV systolic dysfunction but strain and strain rate failed to discriminate patients with TM with elevated NT-pro-BNP from those with normal ones.en_US
