dc.description.abstract[EN] Chronic periodontitis is one of the most common diseases in the oral cavity that every dentist in everyday practice encounters. Unfortunately, periodontal diseases are often underestimated and neglected in the patient's healing plan. It is characterized as a slowly progressive inflammatory process that in the absence of an adequate healing approach leads to tooth loss. Systemic factors and environmental factors (diabetes mellitus, hormonal imbalance, blood disorders, smoking) can alter the response of the immune system to the dental biofilm so that periodontal destruction progresses more progressively. Currently, the most widely accepted and effective approach to treatment of periodontal diseases is the mechanical removal of bacterial biofilm and toxins from the root surface of the tooth - debridement, with the goal of reducing bacterial adhesion and facilitating cell reattachment. Debridement is accepted as a "gold standard" in non-surgical periodontal therapy and is performed using a variety of manual and machine tools. One of the most active areas of scientific research in periodontal therapy in recent years is the therapeutic use of lasers, the main purpose of which is to replace curettes, scalpels and other tools. Currently, lasers in periodontitis are used for surgical procedures such as gingivectomy and extension of the clinical crown, non-surgical periodontal therapy and for the removal of pathologically altered tissues. Two of the supposed benefits of using laser-mediated periodontal therapy are subgingival curettage and microbial count reduction. To date, however, the view of consensus in the many systematic reviews and meta-analyses published over the past ten years is that there is insufficient evidence to show significant improvement in clinical parameters compared to conventional SRP. This has led to the need for further investigation of high-energy lasers and their use in non-surgical periodontal therapy.en_US
dc.description.abstract[BG] Хроничният пародонтит е едно от най-широко разпространените заболявания в устната кухина, с което се среща всеки лекар по дентална медицина в ежедневната си практика. Характеризира се като бавно прогресиращ възпалителен процес, който при липса на адекватен лечебен подход води до загуба на зъби. Системни фактори и фактори от околната среда могат да променят отговора на имунната система към денталния биофилм, така че пародонталната деструкция да протича по-прогресивно. В момента най-широко приетият и ефективен подход за лечение на пародонталните заболявания е механичното отстраняване на бактериалния биофилм и токсините от кореновата повърхност на зъба - дебридмънт, като целта е да се намали бактериалната адхезия и да се улесни клетъчното реприсъединяване. Дебридмънтът е приет за " златен стандарт " в нехирургичната пародонтална терапия и се осъществява с помощта на редица ръчни и машинни инструменти. Една от най-активните области на научни изследвания в пародонталната терапия през последните години е терапевтичната употреба на лазери като основното им предназначение е да заменят кюретите, скалпелите и други инструменти. Понастоящем лазерите в пародонтологията се използват за хирургични процедури като гингивектомия и удължаване на клиничната корона, за нехирургична пародонтална терапия и за отстраняване на патологично променени тъкани. Към днешна дата не са налице достатъчно доказателства, които да показват значително подобрение в клиничните параметри в сравнение с конвенционалния SRP. Това доведе до необходимостта от допълнително изследване на високоенергийните лазери и приложението им в нехирургичната пародонтална терапия.BG
