dc.description.abstractТумори на надбъбречната жлеза се откриват сравнително често при образни изследвания на коремната кухина по друг повод. Малък процент са хормонално активни, числят се към редките заболявания и създават специфични предизвикателства пред анестезиологичния и хирургичния екип. Цел на нашето ретроспективно проучване е да установим разликите в хемодинамиката и използваните техники за контрол по време на обща анестезия за лапароскопска адреналектомия при различните тумори на надбъбречната жлеза. Изследваната популация се характеризира с висок коморбидитет като по-голямата част от пациентите се представят предоперативно с хипертония, а тези с феохромоцитом и с кардиомиопатия. Интубацията и манипулацията върху жлезата непосредствено преди клампажа на венозните съдове са критични моменти по отношение на хипертензията при пациенти с феохромоцитом. При всички пациенти, подложени на адреналектомия се установява интраоперативна хемодинамична нестабилност, но при тези с феохромоцитом е значително по-изразена, с поява на екстремна хипертония и по-висока честота и продължителност на хипотонията. Честотата на приложение на изследваните техники за хемодинамичен контрол е значително по-висока при пациентите с феохромоцитом, в сравнение с останалите групи. Практиката в нашия център отговаря на публикуваните в литературата препоръки и алгоритми като различия се откриват само с част от авторите по отношение обема на хемодинамичния мониторинг и някои от прилаганите медикаменти.BG
dc.description.abstractAdrenal gland tumours are found relatively often on imaging studies of the abdominal cavity on other occasions. A small percentage are hormonally active, considered rare diseases, creating specific challenges for the anaesthesiology and surgical team. Our retrospective study aims to determine the differences in haemodynamics and control techniques used during general anaesthesia for laparoscopic adrenalectomy in different adrenal tumours. The studied population is characterized by high comorbidity. The majority of patients are presenting preoperatively with hypertension. Those with pheochromocytoma also have cardiomyopathy in a high percentage. Intubation and gland manipulation immediately before venous clamping are critical moments regarding hypertension in patients with pheochromocytoma. Intraoperative haemodynamic instability was found in all patients undergoing adrenalectomy, but in those with pheochromocytoma, it was significantly more pronounced, with the occurrence of extreme hypertension and a higher frequency and duration of hypotension. The application of different techniques for haemodynamic control was significantly higher in patients with pheochromocytoma compared to the other groups. The practice in our centre corresponds to the recommendations and algorithms published in the literature. Differences are found only with some authors regarding the volume of haemodynamic monitoring and some of the administered medications.en_US
