dc.description.abstractНастоящото проучване показва, че имунофенотипизирането като модерен и прецизен метод, използван в съвременната имунология чрез изследване на поредица от имунологични показатели, допринася значително за сравняване и проследяване промените в имунната реактивност при болните с хронична бъбречна недостатъчност, на периодична хемодиализа и след бъбречна трансплантация. Определянето на по-важните фактори, които въздействат върху клетъчно-медиирания и хуморален имунитет -бактериални и вирусни инфекции; композиция на диализната мембрана; начин на съдовия достъп; имуносупресивната терапия; продължителност на диализната лечение рЕПО; хепарин, етиология на основното заболяване, е необходимо за точното установяване и своевременно повлияване на нарушенията му имащи значение за прогнозата и качеството на живот на бъбречно болните. Установяването и проучването на по-главните фактори, повлияващи реактивността води до по-доброто разбиране на главните и патогенетични механизми, отговорни за защитата на организма. Те са предпоставка за изработването на нови видове ваксини, внедряването и модифицирането на по-биокомпатибилни диализни мембрани, хемосъвместими и резистентни на бактерии диализни катетри, както и съвременни успешни стратегии в терапията на имунните и автоимунни заболявания. Така ще се преустанови прогресирането на имунологичните нарушения с ХБН, на ПХД и след БТ и ще се подобри тяхната преживяeмост.BG
dc.description.abstractThe study presents immunophenotyping as an advanced and precise method used in modern immunology by testing series of immunological parameters. It contributes significantly to comparing and monitoring changes in immune reactivity in patients with chronic kidney disease (CKD) on recurrent haemodialysis and after renal transplantation. Establishing important factors that affect cell-mediated and humoral immunity - bacterial and viral infections; composition of the dialysis membrane; vascular access; immunosuppressive therapy; duration of dialysis treatment with EPO; heparin; aetiology of the underlying disease, is necessary for the accurate identification and timely response to its deviations, relevant for the prognosis and quality of life of the CKD patients. Determining and studying the major factors affecting reactivity contribute to a better understanding of the main and pathogenetic mechanisms responsible for human defence system. They are a prerequisite for the development of new types of vaccines, the implementation and modification of bio-compatible dialysis membranes, chemically-compatible and bacterially resistant dialysis catheters, as well as successful strategies in the treatment of immune and autoimmune diseases. This will stop the progression of iimmunological disorders in patients with chronic kidney disease, recurrent haemodialysis and after renal transplantation, and improve their survival rate.en_US
