dc.description.abstractНаучната цел на дисертационния труд е да се проучат, анализират, приложат и оценят класическите и съвременните хирургични техники за риносептопластика, като въз основа на нашия опит да се предложат иновативни хирургични техники и алгоритъм за тяхното прилагане. За двадесет годишен период /1994 – 2015/ сме изследвали и оперирали 529 пациенти, от които с пълна документация са 352 оперирани: Жени – 240 /68,1%/, Мъже – 112 /31,9%. Средната възраст при жените бе 26,6 години, а при мъжете 29,4 години. Най-младият е дете на 16 години, а най-възрастният пациент е жена на 73 години. Най-малкият пациент, при когото сме приложили външен достъп, но при тумор, е дете на 1 година и 3 месеца. Риносептопластика с външен достъп се извърши при 214 пациенти /60,8%/, Риносептопластика с ендоназален достъп се извърши при 136 пациенти /38,6%/, Риносептопластика с комбиниран достъп се извърши при 2 пациенти /0,6%/ Изводи: Съвременните тенденции са в посока на комбинация между класическите редуциращи риносептопластични процедури и съвременни уголемяващи хирургични техники с използването на присадки. Въведеният от нас външен достъп при риносептопластика, макар и по-труден от ендоназалния, е предпочитана хирургична техника, когато става въпрос за корекция на по-значими деформитети. Ендоназалният и отворен достъпи не следва да бъдат противопоставяни един на друг, а по-скоро да се прецени за всеки конкретния случай кой достъп да бъде приложен. Това зависи от обективните показания, от оценката на риска и от опита на хирурга, а дори и от желанието на пациента.BG
dc.description.abstractThe aim of this study is to compare classical and modern rhinoseptoplasty techniques based on 20 years of experience. The contingent of the research are 352 patients, operated by us using rhinseptoplasty: 240 /68.1%/ women and 112 /31.9%./men. External septorhinoplasty was performed to 214 patients /60.8%/ and septorhinoplasty with endonasal approach was performed to 136 /38.6%/of the patients and to 2 patients /0.6%/ blended approach. We provided external (open) approach to patients with heavy deformities, i.e. “difficult nose”. We systematized the main disadvantages of open approach in external septorhinoplasty as follows: - It is not always possible to provide a wide approach to the nasal structures with adequate visibility. - It is not always possible to perform correctly some rhinoplastic procedures, for example suture methods of the alar cartilage, re-implantation and its fixation. - When operating the so-called “difficult noses” the surgeon faces old zones of coalescence and cicatricial tissue and profuse bleeding, this makes the dissection and rhinoplastic procedures more difficult. On the basis of our surgical experience we reached the following conclusions Septorhinoplasty with external approach is easier to perform than the endonasal approach. We recommend that septorhinoplasty with external approach is performed on the “difficult noses” – with serious traumatic deformities and re-operations in septorhinoplasty. Rhinoplasty with endonasal approach is still important and is a routine method for correction of the external nose. In conclusion, it has to be pointed out that there is not one and only, or a best technique. The proper approach should be consistent with the unique features of the patient, with surgeon’s expertise and even with patient’s preferences.en_US
