dc.description.abstractОстри и възпалителни заболявания на средното ухо засягат все по-често децата и възрастните в световен мащаб и у нас. При възстановителните операции по повод на хроничен среден отит чрез осикулопластика на слуховата верига се използват чужди ресторативни биоматериали, които са доста скъпи. Това налага необходимостта от разработване и своевременно внедряване на оригинална българска протеза за осикулопластика, която да бъде биопоносима, с достатъчно добри биофизични и биоакустични характеристики и с достъпна цена. Проведени са морфологични и физикомеханични проучвания на оригинално разработени осикуларни протези от различни биоматериали, показващи предимствата на използвания досега само в стоматологичната протетична практика церомер Targis. Основните му предимства са следните: липса на хидрофилност и корозивност, много добра биопоносимост, биостабилност и звукопроводимост; отлични биофизически качества (маса, плътност и ригидност). Протезата се характеризира с добра повърхност, доказана чрез сканираща електронна микроскопия и гарантираща благоприятен интерфейс между нея и реципиента; с по-лесна обработваемост и по-сполучливо моделиране в сравнение с хидроксилапатита; с ниско собствено тегло при дълготрайна еластичност и ниски вътрешни напрежения. Липсват екструзии и фиксации на протезата към околните структури в средното ухо. Липсва резонанс при най-важните говорни честоти (500-2000 Hz).Те обективизират подобреното индивидуално качество на живот на тези болни. Икономическата ефективност на протезата е висока.BG
dc.description.abstractAcute and chronic inflammatory diseases of the middle ear affect more commonly children and adults all over the world, Bulgaria not being an exception. Foreign restorative biomaterials which are very expensive are being used for reconstructive operations by means of ossiculoplasty of the ossicular chain on the occasion of chronic otitis media. This calls for the elaboration and timely implementation of an original Bulgarian prosthesis for ossiculoplasty that should be biocompatible and present with sufficiently good biophysical and bioacoustic characteristics as well as of accessible price. A series of morphological and physico-mechanical investigations of originally created ossicular prostheses from different biomaterials such as gold, teflon and hydroxylapatite have been performed. They convincingly demonstrate the advantages of the ceromer Targis used previously in the dental prosthetic practice only. Its main advantages are as follows: absent wetting ability and corrosiveness, very good biocompatibility, biostability and sound-transmission; excellent biophysical properties such as mass, density and rigidity. The prosthesis is characterized by a good surface that is proved by scanning electron microscopy. This prosthesis warrants a favourable interface between it and the recipient; by easier processing and better modelling in comparison with hydroxylapatite; by lower own weight along with long-lasting elasticity and low internal tensions. There are neither extrusions, nor fixations of the prosthesis to neighbouring middle ear structures. There is no resonance at the most significant speech frequencies of 500-2000 Hz. The audiometric and inquiry examinations objectify the improved individual quality of life of the patients with implanted Targis prostheses. The economic effectiveness of the prosthesis is sufficiently high.en_US
