dc.description.abstractИнфекциозната диария е една от причините за заболяемост и смъртност в детска възраст. Напоследък се наблюдават положителни ефекти от приложението на цинк и при пациенти без цинков дефицит с диария. Налице е повсеместно разрастване на недостига на витамин D, включително в ранна детска възраст, което съвпадна с разкриването на редица негови некалцитропни действия, едно от които е поддържане на лигавичните бариерни функции срещу инфекциозни агенти. Целта е да се установи цинковия и витамин D статус на деца във възрастта от 3 месеца до 3 години и 6 месеца с остра диария, и да се оцени ефекта от приема на цинк, като допълнение към лечението. В проучването са включени 167 деца в горепосочената възраст, 30 от които здрави контроли, а останалите 137 - пациенти с остра диария, лекувани в Инфекциозна клиника - Варна в периода 06.2010-06.2013г. Извършиха се: антропометрични измервания, клинико-епидемиологично проучване, изчисляване количеството цинк в диетата, определяне на серумния цинк, 250HD3 на участниците. Цинков препарат в доза Юмг за денонощие получиха 66 пациенти, останалите 71- плацебо. Резултатите се обработиха чрез вариационен и корелационен анализ. Установи се значимо по-нисък серумен цинк при болните, в сравнение със здравите контроли. Цинковият статус показа зависимост от възрастта и количеството на цинк в диетата. Субогпимален витамин D статус имат 66% от изследваните, с тенденция за по-ниски 250HD3 нива при децата с недостатъчен прием на цинк с диетата. Установи се, че приемът на цинк предотвратява срив на серумните му нива по време на диария, намалява клинично тежестта на епизода, значимо по-изразено при пациентите над 12 месеца, при колитен синдром, и при доказана инфекциозна етиология на заболяването.BG
dc.description.abstractInfectious diarrhea is a leading cause of childhood morbidity and mortality. Zinc administration during diarrhea has shown positive results, even in patients without zinc deficiency. Increase of vitamin D deficiency state is observed worldwide, including young infants. This phenomenon coincides with the discovery of many non-classical actions of the vitamin, among which maintenance of the mucosal barrier function against microbes. The aim of this research was to establish the zinc and vitamin D status of young children with acute diarrhea and to assess the effect of zinc therapy. The study contingent was 167 infants in the age 3-42 months. The control group consisted of 30 healthy children, the rest 137 were patients with acute diarrhea, treated in the Clinic of Infectious Diseases – Varna, in the period 06.2010-06.2013. The following activities took place: anthropometric measurements, clinical research, calculation of the daily zinc amount in the patients’ diet, establishment of the participants’ serum zinc and 250HD3. Sixty six patients received 10 mg of zinc gluconate daily through the hospital stay, the rest 77 - placebo. Variation and correlation analysis were used for statistical processing. Serum zinc of the patients was found to be significantly lower compared to healthy controls. Zinc status showed dependency on the age of patients and on its amount in their diet. Suboptimal vitamin D status had 66% of the examined. A tendency for lower serum 250HD3 in those with suboptimal amounts of zinc in the diet was established. Zinc administration through diarrheal episode was found to prevent a dramatic drop in serum zinc, decrease of the episode severity, markedly in patients above 12 months of age, with colitis, and proved infectious etiology of the disease.en_US
