dc.description.abstractИзследвани 57 доброкачествени, шест атипични и 24 малигнени пигментни лезии, с имунохистохимичен анализ на Cyclin D1, BCL2, P53, S100, HMB45 и Ki 67, по единадесет моделна схема за оценка на експресията, детайлизирана по топография и интензитет, за търсене на маргинално противоположни резултати, между доброкачествените и злокачествените тумори, които да послужат за предикция на малигнен потенциал. Използвани рутинни хистологични и имунохистохимични методи на изследване, с реактиви на DAKO и червен хромоген. Статистически методи за обработка на резултатите: средна аритметична величина, х-квадрат анализ на Pearson, ANOVA тест, Kruskal – Wallis тест, Error Bar graphs, Post – Hoc тест по Tukey и Dunn, корелационен анализ по Spearman и ROC крива анализ. Базирано на резултатите формулирахме следните изводи: Липсата на HMB45 градиентна експресия при меланомите e прогностичен белег за малигненост. Разлика в експресията на Cyclin D1 между групите позволява употребата му за предикция на малигнитет. Разликата в р53 експресията между групите го прави подходящ за предиктор само в комбинация, поради установената в макар и малка част от доброкачествените лезии и атипичните невуси имунореактивност. BCL2 е неподходящ за предиктор на малигнитет, поради широко припокриване на експресията между групите. Пролиферативната активност при пигментните неоплазии, не може да бъде индикатор за малигнен потенциал, поради широкия си диапазон на проява при тези тумори. Разумният подход в прилагане на имунохистохимичните маркери за предсказване на малигнен потенциал изисква използването им в комбинация - според нашите резултати Cyclin D1, p53 protein и Melanosome HMB 45 с висока прогностична стойност при пигментните неоплазии.BG
dc.description.abstractA total of 57 benign, six atypical and 24 malignant pigment lesions were studied, with immunohistochemical analysis of Cyclin D1, BCL2, P53, S100, HMB45 and Ki 67, according to an eleven model scheme for estimating expression, detailed by topography and intensity, for differential results, between benign and malignant tumours, which serve to predict malignant potential. Routine histological and immunohistochemical examination methods were used, with DAKO reagents and red chromogen. Statistical methods for processing the results: arithmetic mean, x-square analysis of Pearson, ANOVA test, Kruskal - Wallis test, Error Bar graphs, Post - Hoc test by Tukey and Dunn, correlation analysis by Spearman and ROC curve analysis. Based on the results, we formulated the following conclusions: The lack of HMB45 gradient expression in melanoma is a prognostic sign of malignancy. The difference in the expression of Cyclin D1 between the groups allows its use to predict malignancy. The difference in p53 expression between the groups makes it a suitable predictor only in combination, due to the immunoreactivity found in even a small proportion of benign lesions and atypical nevi. BCL2 is unsuitable as a predictor of malignancy due to the wide overlap of expression between groups. Proliferative activity in pigmented neoplasms cannot be an indicator of malignant potential due to its wide range of manifestations in these tumours. A reasonable approach in the use of immunohistochemical markers to predict malignant potential requires their use in combination - according to our results Cyclin D1, p53 protein and Melanosome HMB 45 with high prognostic value in pigmented neoplasms.en_US
