dc.description.abstractЦелта е да се изследват ранните микроструктурни промени в макулата и перипапилерния неврофибрилерен слой при болни от захарен диабет, като се вземат под внимание рискови фактори за развитие на диабетна ретинопатия. За постигането ù са изследвани 212 лица (92 мъже, 120 жени). Пациентите са разпределени в две групи: I контролна група- 75 здрави лица (148 очи) без наличие на ЗД; II група- 137 лица с наличие на захарен диабет (248 очи)- II A подгрупа: болни от ЗД, без ДР- 71 пациенти (140 очи) ; II B подгрупа: болни от ЗД, с лека НПДР - 66 пациенти (108 очи). Пациентите са изследвани с помощта на Zeiss Cirrus 5000 HD-OCT. Резултатите от проведеното проучване дават основание да се отхвърли хипотезата, че невродегенеративните промени в ретината при ЗД настъпват преди микроваскуларните изменения и да се потвърди теорията, че двете събития са взаимосвързани и настъпват успоредно. С помощта на ОСТ-А е установено, че индексът на циркулярност на фовеоларната аваскуларна зона, следван от съдовата и перфузионна плътност в повърхностният капилярен плексус са най-надеждните показатели за ранни микроциркулаторни увреждания при диабетна ретинопатия. Потвърди се, че давността на захарния диабет и качеството на гликемичния контрол са най-значимите рискови фактори за поява на диабетна ретинопатия.BG
dc.description.abstractThe thesis aims to study the early microstructural changes in the macula and peripapillary neurofibrillary layer in patients with diabetes mellitus, taking into account risk factors for the development of diabetic retinopathy. To achieve this aim, 212 people (92 men, 120 women) were included in the study. Patients were defined in two groups: I control group - 75 healthy individuals (148 eyes) without the presence of diabetes; II group - 137 persons with diagnosed diabetes (248 eyes) - II A subgroup: patients with diabetes, without DR - 71 people (140 eyes); II B subgroup: patients with the diagnose diabetes mellitus, with mild NPDR - 66 people (108 eyes). Patients were examined using the Zeiss Cirrus 5000 HD-OCT. Based on the study results, the hypothesis that neurodegenerative changes in the retina in diabetes occur before microvascular changes were rejected. The theory that the two events are interconnected, and parallel has been confirmed. The circular index of the foveolar avascular zone, followed by vascular and perfusion density in the superficial capillary plexus, is the most reliable OCT-A indicator for early microcirculatory damage in diabetic retinopathy. It has been confirmed that the age of diabetes and the quality of glycemic control are the most significant risk factors for the development of diabetic retinopathy.en_US
