dc.description.abstractЦел: Определяне на характеристиката и прогнозата на хроничната сърдечна недостатъчност (ХСН) при проспективно проучване на пациенти, в контекста на общата оценка за действието на определени биомаркери и прогностични фактори. Материал и методи: Проучването е проведено върху 228 последователно, хоспитализирани пациенти с ХСН и 24 здрави контроли. Всички пациенти са изследвани със стандартизиран протокол включващ вземане на кръвни проби за измерване на Мозъчен натриуретичен пептид (BNP) в серум, Матриксна металопротеиназа-9 (MMP -9) и Хемоксигеназа - 1 (HO -1) в плазма. Пациентите с ХСН са проследени за период от 24 месеца за възникването на рехоспитализация по повод на декомпенсирана ХСН и обща смъртност. Резултати: Честотата на ХСН при пациенти под 65 г. и жени е висока. Исхемичната болест на сърцето е основен етиологичен фактор. Пациентите с ХСН и инструментални данни за белодробен застой, хипотония и предсърдно мъждене са с периситиращо високи BNP стойности при изписването. Установените зависимости на ММР-9 и НО-1 с ЛК функция и дилатация, съответно определят ролята им в маркиране на процесите на матриксно ремоделиране и оксидативен стрес. Честотата на усложнения в периода на проследяване е висока, но съответства на установените показатели в средно- и западноевропейските страни. Функционалният клас е най-силният и независим прогностичен фактор, както за рехоспитализация по повод на декомпенсирана ХСН, така за формиране на общата смъртност в хода на дефинирания период на проследяване и при изследваните от нас пациенти.BG
dc.description.abstractThis thesis sets itself the task to determine the characteristic and prognosis of chronic heart failure (CHF) in prospective study on patients including the assessment of the role of the investigated biomarkers and prognostic factors. Study design: The study is conducted on 228 hospitalized consecutive patients with CHF. The complications during the follow up were defined as readmission due to de-compensated HF and total mortality. Brain natriuretic peptide BNP was investigated on 85 patients. BNP, Metalloproteinase - 9 (MMP -9) and Hemoxygenase - 1 (HO -1) were measured on 59 and 53 patients, respectively at discharge. Values of MMP-9 and HO-1 were compared with the measured levels of both enzymes in 24 healthy controls (age range 47 to 82 years, mean age 68.7 7.9). Results: Ischemic heart diseases were the main etiologic factor for CHF. Frequency of CHF among the investigated women was high. Leading etiologies for CHF among women were hypertension and valvular heart disease. Patients with instrumental data for pulmonary congestion, arterial hypotension and atrial fibrillation were with persistently high BNP values at discharge. Plasma HO-1 were lower in patients with CHF as compared to healthy controls and correlated independently with the degree of LV dilatation. Plasma was independent on the neurohormonal changes in CHF. Patients with CHF were found to be with higher plasma MMP-9 as compared to healthy controls. MMP-9 was increased in CHF, independently on etiology of HF and BNP. Mortality at the end of the study is 26.3. Rate of readmissions due to HF de-compensation is 37.7. Age is statistically significant predictor for mortality. Functional class is the main, independent prognostic factor for risk of complications over the follow up and among the investigated patients.en_US
