Лапароскопската радикална простатектомия през последното десетилетие се наложи като златен стандарт при оперативното лечение на карцинома на простата. Целта на настоящата работа е да обективизира хирургичната травма чрез системната реакция на организма, на базата на променените нива на (CRP, SAA, IL - 6, IL - 10) при пациенти подложени на лапароскопска и отворена екстраперитонеална ретропубична простатектомия. ПАЦИЕНТИ И МЕТОДИ: Бяха проследени 403 пациенти, от които 163 с (eLRP) и 240 с (RRP). Пациентите бяха в един и същ клиничен стадий и различно периоперативно протичане (продължителност на оперативната процедура, усложнения, кръвозагуба, брой трансфузии, среден болничен престой, продължителност на катетеризация) онкологичен статус ( - Gleason score, патологичен стадий, чиста резекционна линия). РЕЗУЛТАТИ: Величината на системният отговор при хирургично индуцираната тъканна травма се проследи при всички пациенти чрез промените на нивата на медиаторите на острата фаза: С-реактивен протеин, серумен амилоид А, интерлевкин - 6, и интерлевкин - 10, преди, по време и след радикалната простатектомия. ИЗВОДИ: Проучването показа, че инвазивността на лапароскопската простатектомия на базата на измерените показатели е сравнима с отворената ретропубчина простатектомия, но обективните критерии засягащи пряко пациента са предимство изцяло в полза на лапароскопската процедура. In the past decade laparoscopic radical prostatectomy established itself as the gold standard in surgical treatment of prostate cancer. The aim of this study is to report a retrospective, controlled non randomized patient systemic response based on revolution of (crp, saa, il - 6, il - 10) levels of the patient after extraperitoneal laparoscopy (eLRP) and open retropubic radical prostatectomy (RRP). PATIENTS AND METHODS: This research is based on the tracking record of 403 patients subjected to eLRP (163) or open RRP (240); patients in both groups had similar preoperative staging in addition to perioperative variables (operative duration, complications, blood loss, transfusion rate, hospitalization, catheterization); oncological data (- Gleason score, pathological stage, positive margins) were also compared. RESULTS: The extent of the systemic response to surgically induced tissue trauma was tracked in all patients by assessing the levels variations of acute - phase markers: C - reactive protein, CRP serum amyloid A (SAA), interleukin - 6 (IL - 6), and IL – 10, before, during and after radical prostatectomy (RP). CONCLUSIONS: Surgery duration, transfusion rate, hospital stay and duration of catheterization are comparable to those in previous studies. There is an increase in IL-6; CRP and SAA, but no change in IL-10 and no differences between eLRP and RRP over the entire period assessed. The invasiveness of eLRP could not be substantiated objectively based on the variables measured in this study. The surgical trauma and associated invasiveness of both methods are equivalent but objective criteria directly affecting patients prevail entirely in favor of the laparoscopic procedure.
